ABSTRACT

Objective:To analyze the relationship between capacity of general practice team leaders and the team performance in community health service centers.Methods:This study was a cross-sectional study. A questionnaire survey was conducted between November and December 2022, among general practice team leaders and general practitioners (GPs) from 18 community health service centers in the urban and suburban areas of shanghai selected by stratified sampling method. The personal information questionnaire, leadership of general practice team leader questionnaire,and work performance scale of general team members were used for the survey. The relationship between leadership of general practice team leaders and team performance was analyzed based on structural equation model (SEM).Results:A total of 944 questionnaires were distributed and 856 valid ones were returned with a response rate of 91.0%, including 110 general practice team leaders and 749 were GPs. The SEM analysis showed that some dimensions of the management ability of the general practice team leader had significant effect on the employee organization loyalty (organizational management: β=0.37, teaching and research management: β=-0.29, strategy and cultural construction: β=0.23, personal quality: β=0.11) and work performance (special business management: β=0.95, organizational management: β=0.54) (all P<0.05); and employee organization loyalty played a partial mediator role in relationship between leadership of general practice team leaders and work performance with a mediating effect of 39.50%. Conclusion:The management ability of the general practice team leader directly affect or indirectly affect the work performance of team members through team members′ organizational loyalty.

ABSTRACT

Objective:To establish selection criteria of community mentors for postgraduates in general medicine.Methods:A subject index system of selection criteria of community tutors for general practice postgraduates was drafted and refined based on literature retrieving and in-depth interviews during January to December 2020. Twenty experts in general practice related fields were invited for two rounds of Delphi consultation. Analytic hierarchy process was used to calculate index weight coefficient and consistency test; and the developed selection criteria were evaluated.Results:Through the in-depth interviews with 11 general practice faculty and 11 general practice postgraduates, the following five first-level thematic frameworks were refined: (1) the professional qualities; (2) the basic qualities; (3) the clinical ability; (4) the teaching ability; (5) the scientific research ability. After two rounds of Delphi expert consultation, the selection criteria of community tutors for postgraduates in general practice were finally constructed, including 5 first-level indicators (professional quality, basic quality, medical practice ability, teaching and guidance ability, academic research ability) and 28 second-level indicators. The positive coefficients of experts were 85% and 100%; the expert authority coefficients were 0.825 and 0.860; and the expert opinion coordination coefficients were 0.486 and 0.515, respectively (all P<0.05). the weight coefficients of the five first-level indicators were 0.345, 0.210, 0.138, 0.210 and 0.097, respectively. Conclusion:The criteria and index system on the selection of community mentors for general practice postgraduates has been successfully developed in the study.

ABSTRACT

Objective:To survey on the status quo of the management capabilities of general practice team leaders in community health service centers in Shanghai.Methods:Using stratified sampling method, 18 community health service centers in the urban and suburban areas of Shanghai were selected, then the general practice team leaders and general practitioners (team members) in the centers were selected as the study participants. The questionnaire survey on the management capabilities of general practice team leaders was conducted among selected participants from November to December 2021. The self-assessment by general team leaders and the other-assessment by team members were carried out, and the total scores was calculated according to the weight of self-evaluation score (30%) and other-evaluation score(70%).Results:A total of 110 general practice team leaders and 749 team members participated in the survey. Among the team leaders, 63.64% (70/110) were females, 82.73% (91/110) were bachelor degree holders, 71.82% (79/110) had intermediate professional title, and 50.91% (56/110) worked for more than 9 h per day. The total score of general practice team leaders was (87.15±10.76) points, the other-assessment score was significantly higher than self-assessment score ((88.62±9.34) vs.(83.71±14.08), t=2.22, P<0.05). The average score of the 5 dimensions was 4.41 for special business management, 4.39 for organizational management, 4.38 for personal quality, 4.27 for teaching and research management and 4.22 for strategy and cultural construction, respectively. Conclusion:The overall management capabilities of Shanghai community general practice team leaders are at the upper-middle level, however, the capabilities in scientific research and team planning are relatively weak.

ABSTRACT

Abstract Background: Mothers' perceptions of the quality and quantity of care they have received and their relationship with healthcare personnel significantly impact continuity of care and future referrals. This study evaluated the psychometric dimensions of the Iranian version of the Mothers' Perception of Postpartum Health Care questionnaire in comprehensive health centers. Methods: We conducted a cross-sectional study in which the questionnaire was administered to 250 women from different comprehensive health centers. First, the questionnaire was translated verbatim from English to Persian. Then, we evaluated the face, content, structural validity, and reliability of the questionnaire. Results: The results showed that this questionnaire's face, content, structure validity, and reliability were adequate. Cronbach's alpha coefficient for the entire questionnaire was 0.668. The interclass correlation coefficient was 0.688, which confirmed divergent validity. Conclusions: Mothers' Perception of Postpartum Health Care questionnaire in comprehensive health centers is valid and reliable for measuring maternal perception of these vital services.


Resumen Introducción: La percepción de las madres con respecto a la calidad y cantidad de la atención que han recibido, así como de su relación con el personal de salud impactan significativamente en la continuidad de la atención y sus futuras derivaciones. Este estudio se diseñó para evaluar las dimensiones psicométricas de la versión iraní del Cuestionario de Percepción de las Madres sobre la Atención de la Salud Posparto en los centros integrales de salud. Métodos: Se llevó a cabo un estudio transversal en el que se aplicó el cuestionario a 250 mujeres de distintos centros integrales de salud. En primer lugar, el cuestionario se tradujo de forma literal del inglés al persa. Después se evaluó la validez aparente, de contenido y de estructura, así como la confiabilidad del cuestionario. Resultados: Los resultados mostraron que la validez aparente, de contenido y de estructura, así como la confiabilidad de este cuestionario son adecuadas. El coeficiente alfa de Cronbach para todo el cuestionario fue 0.668. El coeficiente de correlación intraclase fue 0.688, lo que confirmó una validez divergente. Conclusiones: El Cuestionario de Percepción de las Madres sobre la Atención de la Salud Posparto en los centros integrales de salud es válido y confiable para medir la percepción materna sobre estos servicios vitales.

ABSTRACT

Objective:To investigate the status quo of nursing capacity for chronic respiratory disease in community health service centers.Methods:A questionnaire survey on nursing capacity for chronic respiratory disease was conducted in November 2018 among nurses working in internal medicine, general practice and chronic disease management departments of 4 community health service centers in Beijing Miyun District. The questionnaire included the knowledge and work content of chronic respiratory diseases. At the same time semi-structured interviews was conducted on the role of nurses in the management of chronic respiratory diseases among nurses selected by purposive sampling method.Results:In the study 63 valid questionnaires were collected and 14 nurses were interviewed. The results showed that the total score of nurses′ knowledge about chronic respiratory diseases was (62.0±14.4), and the scores of knowledge of chronic obstructive pulmonary disease (COPD), bronchial asthma and obstructive sleep apnea were (55.5±19.3), (61.0±21.6), (69.5±25.1), respectively. The total knowledge score of nurses with middle-rank professional title or above was higher than that of those with primary title [(67.0±12.9) vs. (54.0±13.6), t=-3.07, P<0.01]. The work contents mainly involved were nursing procedures(22/63,34.9%), health education(18/63,28.6%) and nursing evaluation(8/63,12.7%).Three themes were extracted from the deep interviews: the main work contents for nurses should be nursing procedures, nurses should participate in the chronic respiratory disease management as a member of the integrated medical team; nurses expected practice-oriented trainings of chronic respiratory disease management. Conclusions:The knowledge of chronic respiratory diseases among nurses in community health center needs to be improved, and the nursing skills special for respiratory diseases are needed. It is suggested to develop training courses and nursing guidelines suitable for community health centers and for nurses to participate in chronic respiratory disease management as team members, so that nurses in primary care institutions can play a better role in the management of chronic respiratory diseases.

ABSTRACT

Objective:To investigate the status quo and related issues of reception of pediatric patients by general practitioners in community health centers in Longhua District of Shenzhen through qualitative and quantitative studies.Methods:In qualitative study, 10 general practitioners (GPs) from community health centers in Longhua District of Shenzhen were selected by the purpose sampling method for personal in-depth interviews. The interview contents were recorded and transcribed, then coded by Nvivo software. Theme frame analysis method was used for data sorting and theme extraction. Based on the results of qualitative interviews, the questionnaire was developed and distributed through the electronic questionnaire platform. The contents of the questionnaire included general information, the situation of pediatric patient reception in community health centers, the willingness and ability of GPs to receive children, the existing problems and the measures to promote receiving pediatric patients in community health centers.Results:The results of personal in-depth interviews with 10 GPs showed that the obtained information reached saturation. Through repeated reading, induction and analysis of the interview data, four themes were extracted: ① GPs in community health centers received a large number of pediatric outpatients with common diseases; ② GPs in community health centers are willing to receive child patients, but they were lack of ability; ③ there were many problems of pediatric patients reception in community health centers; ④ training and incentive policies would help to promote child patient reception in community health centers. A total of 223 electronic questionnaires were collected, 219 of which were valid for analysis. All the community health centers in Longhua District received pediatric patients, the number accounted for 20%-60% of the total outpatients; 84.5%(185/219)of the responders were willing to receive child patients, 65.8% (144/219)of them assumed that the consulting ability was average. The main problems for GPs to receive pediatric patients were "insufficient training" (76.7%,168/219) and "lack of consulting ability" (55.3%,121/219). The important measures to promote GPs to receive child patients were to improve their own clinical competence (94.1%,206/219), to have clinical faculty with pediatric background mentoring at the community health center (74.9%,164/219), to improve the supporting facilities (42.9%,94/219) and continuous professional support (38.8%,85/219).Conclusion:The status quo of pediatric patient reception in the community of Shenzhen Longhua District is generally satisfactory. The main problems in this aspect are insufficient training for general practitioners and lack of clinical competence, which should be strengthened and improved in the future.

ABSTRACT

Objective:To analyze the career development path and related problems of general practitioners (GPs)in the community.Methods:In October 2019, general practitioners with different positions working in Malu Town Community Health Service Center of Shanghai Jiading district were selected by typical sampling to participate in a focus group interview. Based on the saturation principle, 5 general practitioners were invited for in-depth interviews on their career development and related issues. The original data were analyzed with Colaizzi′s seven-step analysis method.Results:The career development of the general practitioners working in the community was divided into three stages: start stage, experience-accumulation stage and career-differentiation stage. In the start stage (the first 0-24 months), the GPs mainly got familiar with the community health service center through community rotation. In the experience-accumulation stage (1-5 years after entering the community health service center), GPs accumulated clinical experience from outpatient clinic, ward, and being family doctor, etc; at the same time, they accumulated management experience through part-time public health management, research and education management, secondment in district health committee. In the professional differentiation stage (5 years or more after entering the community health service center), GPs may take a position of full-time management, or department head. The three respondents had confusion in certain career development stages, including confusion in clinical skills upgrading and balance between clinical and management duties.Conclusions:The study suggests that general practitioners in the entry stage need to be given timely clinical rotation as well as research and teaching training; in the career differentiation stage, it is necessary to incorporate upgrading clinical service and management capability with policy development directions and institutional development needs to promote the successful career differentiation of general practitioners.

ABSTRACT

Objective:To survey the teaching ability of general practitioners(GPs) in community health centers of Shanghai, and its influencing factors.Methods:A questionnaire survey on the teaching ability of general practice trainers was conducted from January to February 2017. The participants of the survey were GPs with teaching experiences from community health centers in Shanghai selected by a stratified and proportional cluster sampling method. The questionnaire included two aspects: the general information and teaching ability of the participants. The influencing factors of teaching ability were analyzed by univariate and multivariate regression analysis.Results:A total of 1 138 GPs from 116 selected community health centers participated in the questionnaire survey, among whom 63.6% (724/1 138) worked in community teaching bases and 78.5% (893/1 138) had received training for trainers. The total score of teaching ability was 38.00 (30.00, 40.00). The score of the item"interesting in teaching"was (3.46±0.80), 75%—80% of the other questions answered "yes". The score of"application of multi-teaching methods"was (3.78±0.72). Multivariate analyses showed that working for 5—9 and 10—19 years [ OR=7.14 (2.47—11.81), P=0.003; OR=5.32 (1.30—9.33), P=0.009], working in community teaching bases [ OR=13.23 (9.88—16.57), P<0.001] and receiving training for trainers [ OR=17.76 (13.80—21.71), P<0.001] were influencing factors of teaching ability. Conclusion:The main problems related to teaching ability in community GPs are lack of teaching interest and the poor application of multi-teaching methods, the relevant training is necessary for them in the future.

ABSTRACT

Objective:To develop an evaluation index system for the management ability of general practice team leaders in community health service centers.Methods:Based on literature review, key insider interviews and other methods, the framework and indicator pool for management capability of general practice team leaders were formed. Two rounds of Delphi consultations with 15 experts were conducted from July to October 2021, and the evaluation index system of general practice team leaders' management ability established.Results:All 15 experts had at least 5 years of work experience, including 10 engaging in clinical general practice, 3 in public health and 2 in community management. The response rates for the two rounds of expert consultation was 15/15, and the expert authority coefficient was 0.84, with Kendall's W coefficient of 0.35 ( P<0.001) and 0.46 ( P<0.001), respectively. Finally, the evaluation index system of the management ability of general practice team leaders was established, which consisted of 5 primary indicators (personal quality, special business management, organizational management, teaching and research management, strategy and culture building), 11 secondary indicators and 37 tertiary indicators. Conclusions:The management capability evaluation index system general practitioner team leaders of the community health centers has been established in this stu, which may be used for the training and selection of general practice team leaders in the community health centers.

ABSTRACT

Objective:To investigate the frailty state transition in the community-dwelling elderly undergoing health check-up, and related influencing factors.Methods:From September to October 2016, 3 833 residents aged ≥ 60 years undergoing health check-up in Xidu Community Health Center were enrolled in this prospective cohort study by multistage random sampling. All participants completed a questionnaire survey, health examinations, and frailty assessments as measured with the Fried frailty phenotype. A second assessment of frailty status was conducted from June to July 2020, the frailty state transition was analyzed by comparison between two assessments of frailty states. Logistic regression was used to analyze the influencing factors related to frailty progression.Results:The final cohort consisted of 3 061 participants. At the entering of the study, the median age of participants was 71.0 years, 41.0% were male, 1 563 (51.1%) were prefrail, and 156 (5.1%) were frail. At the follow-up, the frailty status of 1 304 (42.6%) participants was progressed and that of 395 (12.9%) participants was improved from their baseline levels, respectively. Logistic regression showed that age ( OR=1.170,95% CI:1.147-1.194), marital status (married: OR=0.377,95% CI:0.292-0.486), physical exercise (never vs. every day: OR=18.610,95% CI:14.461-23.950; sometimes vs. every day: OR=4.210,95% CI:2.186-8.107), baseline frailty state (robust vs. frail: OR=20.464,95% CI:11.779-35.553;prefrail vs. frail: OR=2.147,95% CI:1.270-3.632), stroke ( OR=2.195,95% CI:1.454-3.313) and diabetes ( OR=1.811,95% CI:1.346-2.437) were identified as independent factors influencing frailty progression (all P<0.05). Conclusions:Frailty state progression is common among older adults, and its related factors include age, unmarried status, physical exercise, baseline frailty state, stroke and diabetes. It is necessary to identify elderly at high risk for frailty progression and implement medical interventions.

ABSTRACT

Objective:To investigate the status quo of continuing education and training needs among general practitioners in Pudong New Area of Shanghai.Methods:An online questionnaire survey was conducted among general practitioners registered in the community health service center of Shanghai Pudong New Area in September 2019. The questionnaire included items of general information, the status quo of continuing education, the training needs and demands, and the knowledge and skills to be improved.Results:There were total 1 824 general practitioner in Pudong New Areal, 861 participated in the survey with a participant rate of 47.2%. A total of 861 valid questionnaires retrieved with a response rate of 100.0%. Among all participants, 79 (9.18%) thought that continuing education was very useful for improving or developing their professional ability, 564 (65.51%) thought it was useful, and 41 people (4.76%) thought it was almost useless. There were significant different in views on the roles of continuing education among participants with different ages, types of registration, and working years (χ2=15.544, 15.382, 21.979; P<0.05). There are 112 participants (13.01%) who spend less than 1 day in continuing education each month, and 224 (26.02%) who spent more than 3 days a month in continuing education, the times of attending continuing education was related to the taking administrative duty for general practitioners (χ2=9.551, P<0.05). When general practitioners choose continuing education programs, the first consideration was the practicability of the contents [772 (31.94%)], followed by the form of teaching [541 (22.38%)]. The three most recognized forms of continuing education courses were online learning (28.22%), training and off-line teaching (25.58%) and special lectures (24.69%) The three most required knowledge and skills to be improved through continuing education were the endocrine system, respiratory system, and blood circulation system. Conclusions:The survey shows the current status and traing needs of continuing education among general practitioners in Shanghai Pudong New Area, which indicates that it is necessary to take the job competence as the core and carry out various activities of continuing education for general practitioners in community health service centers.

ABSTRACT

From April to November 2019, 5 community health service centers in Shanghai Hongkou District were randomly selected for thematic group interviews to investigate the current status, problems and challenge for further development of chronic disease management. The community chronic disease management was mainly divided into growth period (typically hypertension and diabetes) and introduction period (cardiovascular and cerebrovascular diseases, respiratory diseases, dementia, osteoporosis). The management of hypertension and diabetes in the growing stage mainly faced difficulties such as refusal or non-cooperation of patients, loss of follow-up, incomplete quality control, not meeting quality control indicators, repeated management of comorbidities, insufficient information sharing, and discrepancy in assessment indicators of multiple parties. The management of cardiovascular and cerebrovascular diseases, respiratory diseases and other diseases in the introduction period mainly faced poor awareness rate of patients, loss of follow-up, difficulty in diagnosis, irregular medication, insufficient information sharing, lack of management plan, insufficient resource investment, and few variety of medicines in the community. The current management system should be refined to provide more effective and efficient chronic disease management in the community. Meanwhile, the awareness of residents, the information network and multi-center collaborative research also need to be strengthened.

ABSTRACT

Objective:To develop a comprehensive ability evaluation system for general practice management post in community health service institutions.Methods:Based on the previous studies and literature search on competency of general practitioners in various clinical posts, the first draft of the comprehensive ability indicator system for general management posts was designed. From November 2019 to January 2020, the comprehensive ability indicator system, classification and scoring method and evaluation method for general management posts were determined through three rounds of Delphi consultation.Results:Seventeen experts with administrative management experiences were invited for consultation, including 3 directors in charge and 14 general practice managers. The response rate of valid questionnaires in the three rounds of consultation were all 17/17, the authority of experts were all>0.7, and the coordination coefficients of expert opinions were 0.142, 0.212, and 0.532 (all P<0.01). The 11 evaluation indexes of the comprehensive ability of community general management posts were: achievement orientation, initiative, interpersonal understanding, service consciousness, influence, organization, training others, teamwork, team leadership, professional skills and confidence. Methods suitable for evaluation the ability of psychological test indicators were achievement orientation, interpersonal understanding, service consciousness and self-confident;appropriate indicators for no-leader group discussion were initiation, influence, team work, team leadership;appropriate indicators for in-tray test ability were organized cognition, professional skills;and the appropriate indicator for the ability of the structured interview was training others. Conclusions:The comprehensive ability evaluation system for community general management posts established in this study is scientific and practical, which may provide objective standard basis for the ability evaluation of community health management talents.

ABSTRACT

Objective:To investigate the status quo of the management of chronic obstructive pulmonary disease (COPD) in primary care health institutions in Miyun District of Beijing.Methods:The Measures to Improve Medical Services in Primary Medical and Health Institutions was issued by Beijing Municipal Health Commission in June 2019. In August 2020, a survey on COPD management status after the measures was conducted with self-designed questionnaire among 19 community health service centers and 61 general practitioners (GPs) from 4 centers in Miyun District of Beijing. Two questionnaires were used for the survey: one was on the status of medical institutions (institution questionnaire, filled by the person in charge of the center) and other was on COPD knowledge of general practitioners (doctor questionnaire, filled by selected GPs). Results:For health institution, 19 valid questionnaires were collected. Among them, 12 centers were equipped with lung function examination equipment after reform; 17 centers were equipped with inhaled corticosteroids/long acting β 2 receptor agonists (ICS/LABA); 12 centers carried out the contract management of family doctors for COPD; 267 patients signed the contract, 7 centers followed up the signed patients at least twice a year; 13 centers referred patients to Miyun District Hospital with 2 024 referrals per year. For GPs, 61 valid questionnaire were collected with a recovery rate of 100% (61/61). Among them 12 GPs (20%) had contracted management for COPD patients; 12 GPs (20%) had referred COPD patients to the secondary or tertiary hospitals; and 9/12 of GPs referred the patients for lung function examination due to the needs of diagnosis or follow-up.The mean COPD-related knowledge score was (58.9±20.0), compared with the score of 65 GPs from the 4 same community centers tested in 2018 (63.2±18.9), there was no significant difference ( t=0.456, P>0.05). Conclusions:The implementation of the primary care institution measures in Beijing promoted the management of COPD and improved capacity building in primary health institutions, also promoted the work docking between primary health institutions and secondary, tertiary hospitals. However, it is still necessary to strengthen the ability training of general practitioners in relevant knowledge and skills to improve their management ability of COPD.

ABSTRACT

Background: The Chilean public health system is based on Primary Health Care (PHC), whose assessment is challenging due to the heterogeneity of services and multi-dimensionality of expected results. The Primary Care Assessment Tool was adapted and validated for Chile. Aim: To analyze, from the provider's perspective, the structure and functioning of the health centers, to determine the achievement of PHC's core functions: access, continuity, coordination, comprehensiveness of care, cultural competence, family centeredness and community orientation. Material and Methods: All professionals working in primary care in a commune of Metropolitan Santiago were invited to answer an online version of the Primary Care Assessment Tool. Results: One hundred and nine professionals (51% of those invited) from four Family Health Centers, two Community Health Centers, and a Community Mental Health Center, answered the online questionnaire. Their distribution by profession and health units does not resemble the whole research population, which should be considered when interpreting the results. Data show a good performance of the system: general and domain specific scores are all near three for a maximum score of four. Family centeredness obtained the highest score, whereas cultural competence had the lowest. Conclusions: Reinforcing intercultural skills and a wider approach to psycho-social problems is recommended to strengthen the new healthcare model implementation.

ABSTRACT

O atendimento a pacientes com necessidades especiais (PNE) na Odontologia exige uma abordagem diferenciada. São relatadas dificuldades desta população em conseguir atendimento, principalmente no serviço público. O objetivo deste estudo foi investigar a percepção e as atitudes dos cirurgiõesdentistas (CD) da rede municipal de saúde de Pelotas/RS no atendimento a PNE. Participaram 47 profissionais de Unidades Básicas de Saúde que responderam a um questionário sobre rotina profissional, tempo de formado, qualificação profissional, experiência, atitudes e dificuldades no atendimento de PNE. Foi realizada análise descritiva e o Teste Exato de Fisher para avaliar associação entre as características do CD, sua percepção, experiências e atitudes frente ao atendimento. A maioria (93,6%) dos CDs relatou atender PNE e a dificuldade mais relatada foi a falta de auxiliar (58,5%). Houve uma melhor percepção em relação ao atendimento ao PNE ente aqueles CD que fazem parte da equipe da Estratégia Saúde da Família, com menos tempo de formado e que se dedicam integralmente ao setor público. Conclui-se que, apesar da maioria dos CD relatarem prestar atendimento odontológico ao PNE, diversas dificuldades foram apontadas, sendo a principal a falta de auxiliar (AU).


The care of special needs patients (SNP) in Dentistry requires a different approach. This population reports difficulties in getting care, especially, at the public service. The aim of this study was to investigate the perception and attitudes of the dentists that work on Public Health Care Centers (PHC)in the city of Pelotas on the care of SNP. A total of 47 professionals from the PHC participated in this research and they answered a questionnaire about their professional routine, time since graduation, professional qualification and experience, attitudes and difficulties related to the care of SNP. Descriptive analysis and Fisher's Exact Test were performed to assess the association between the characteristics of the dentists, their perception, experiences and attitudes towards care. Most dentists (93,6%) reported that they attend patients with special needs and their most difficulty was the lack of a dental assistant to help them (58,5%). There was a better perception regarding the care of SNP among those dentists that are involved in the Family Health Strategy team, with less time since graduation and that are fully dedicated to the public service. We concluded that, although most dentists reported that they attend those SNP, there are several difficulties and the most one is the lack of help (AU).

Under the background of the innovative medical association, the company took the lead in promoting the senior nurses to the community hospitals in Anhui, and opened community nursing clinics to better implement the graded diagnosis and treatment system and meet the needs of residents' integrated health services. From the background of a new round of medical reform, this paper analyzes the new occupational orientation of senior nurses in the innovative medical association model, and introduces and analyzes the problems that arise in the exploration stage of the sinking community, and puts forward suggestions based on the actual situation.

ABSTRACT

Based on literature review and on-site investigation, this paper analyzed the current policies, regulations, supply and demand dilemma and practical mode of such a combination, as well as the feasibility from the views of current policies, service offering and the practice. It is recommended to build a service system centering on " public health services + basic medical care + community elderly/medical support" , with services enriched and supportive mechanisms supplemented, thus transforming community health centers.

ABSTRACT

Acute coronary syndrome (ACS) is a serious type of coronary disease with high morbidity and mortality rates,being one of leading causes of death in both developed and developing countries.Studies abroad have shown that,the cardiac rehabilitation in community hospitals carried out by general practitioners with the guidance of cardiac specialists and the establishment of a continuous,longterm exercise rehabilitation and drug therapy can effectively reduce the mortality of ACS patients.In order to complete the health care chain from prevention,treatment to rehabilitation for ACS patients in China,it is imperative for community hospitals to provide cardiac rehabilitation service.
